If you suspect fraud in Medicaid, Food Assistance, Ohio Works First (including cash assistance and workforce development programs), or Child Care, please fill out the form below or report it to 419-774-7867. You will need to leave a voicemail for our fraud investigators, who will follow up on your complaint.
